REPLACEMENT OF CERTIFICATES AND CARDS
Certificates may be replaced for $13 while membership cards may be replaced for $5. Who pays for the cost of replacement is determined as follows:
If the member has lost the certificate/membership card then the member is responsible for the payment.
If the local chapter made an error in the spelling of the name then the chapter is responsible for the cost of the replacement.
If the printer made an error that can be shown by our records then the certificate/membership card will be replaced with no cost to the member or the chapter.
For replacement of certificates/membership cards the registration form needs to be completed as per instructions on the registration form. Replacement certificate/membership card orders may also be included with a regular order.
